1.Participants in managed care plans that require an authorization for referral to a specialist must bring a referral form from their Primary Care Physician.
2.All Current Insurance Cards.
3.Any updated insurance information or changes in your insurance.
4.Any changes in your personal information, such as change of address, phone number, etc.
5.A list of all medications you take with dosage information and how often you take them. You may bring the actual bottles if you like.
6.Bring any previous records, test results, or X-ray films if they relate to your visit. |
